ED Feedback (edfeedback.com.au) provides a central entry point for staff to access feedback and assessment forms. We take privacy and confidentiality seriously.
Forms linked from this site are hosted on Jotform, a third-party service provider. When you complete a form, information you submit is transmitted directly to Jotform’s servers and is not stored on this website.
You must not include patient names, UR numbers, dates of birth, or any other information that could identify a patient. Feedback and assessment forms are for educational, training, and quality improvement purposes only.
Submitted information is used for clinical education, supervision, training, and quality improvement within Peninsula Health emergency departments. It may be reviewed by supervising consultants, education teams, or relevant administrative staff for these purposes.
Jotform implements industry-standard encryption and access controls. Access to submitted forms is limited to authorised staff with appropriate permissions.
